| | 1. | Students
who wish to apply for University academic scholarships must apply for academic
scholarship at the Office of the Vice President for Academics located at the IRC
Building, 1st level. |

| | 2. | The
applicant will fill in and sign the Application Form and submit the same to the
Office of the Vice President for Academics with the following: |
| | | a.
Certification of Good Moral Character from Barrio Captain or Municipal Mayor; |
| | | b.
Certification of Good Moral Character from Parish Priest or Church Minister; |
| | | c.
2x2 Black and White Picture |
| | | |
| | 3. | The
certification for academic grade (GPA of not less than 1.5) for the previous semester
must be signed by the University Registrar. |
| | | |
| | 4.
| The
Certification for Loyalty and Services rendered must be duly signed by the College
Dean and SAO. |
| | | |
| | 5. | The
applicant must be currently enrolled at the Registrar's Office and must have paid
the pertinent fees at the Treasurer's Office. |
| | | |
| | 6. | The
accepted scholars must attend the Orientation Meeting and other meetings that
will be called by the Vice President for Academics from time to time. |
| | | |
| | 7.
| Acceptance
for application for scholarship will start during enrolment period until the second
week of the start of regular classes. Late applications will not be accepted. |
| | | |
| | 8.
| During
the second semester, the University Academic Scholar renews his/her scholarship
by submitting to the Office of the Vice President for Academics a Certification
of Academic Grade duly signed by the University Registrar, stating that the applicant
has earned a GPA of not less than 1.5 during the previous semester. |
